Resumen:
El presente trabajo pretende reflexionar acerca de las intervenciones asistenciales y las dificultades técnicas con las que se encuentra el profesional en el marco de la atención primaria en salud mental infanto-juvenil. Surge del trabajo y las reflexiones del equipo profesional del Centre de Salud Mental Infantil i Juvenil de Sta. Coloma de Gramenet (Fundació Vidal i Barraquer), y de la necesidad de revisar la teoría psicoanalítica clásica del encuadre, que aplicada al contexto de la atención primaria, más que en ningún otro contexto, se muestra poco útil y nos sitúa en el riesgo de encorsetar la relación terapéutica abocándola al fracaso. Asimismo se propone una reflexión acerca de las condiciones necesarias para flexibilizar el encuadre externo concluyendo en la importancia de un adecuado encuadre interno del profesional de la salud mental como referencia básica y estable ante la inestabilidad de las caóticas situaciones externas con las que habitualmente nos enfrentamos en un servicio de atención primaria en salud mental.

Abstract:
The present paper tries to think brings over the welfare interventions and the technical difficulties which the professional meets in the field of the primary care in mental health. It arises from the work and the reflections of the professional team of Centre de Salut Mental Infantil I Juvenil de Sta. Coloma de Gramenet (Fundació Vidal i Barraquer), and of the need to check the psychoanalytic classic theory of the setting, which applied to the context of the primary care, more that in no other context, proves to be slightly useful beside putting in danger the therapeutic relation. Likewise one proposes a reflection brings over of the necessary conditions to adapt the external setting concluding in the importance of a suitable internal setting of the professional of the mental health as a basic and stable reference before the instability of the chaotic external situations which habitually we face in a service of primary care in mental health.
